But when they happen in medication lists and charts, not so. Even the best are not perfect, and every line item needs review. Having the providers print out a summary of your stay or visit, online and on paper, is crucial. After a few calls and a outright refusal today, we are finally getting things right. She saw two doctors of different specialties in one facility. The first left off two medications from her list, the second listed one no longer taken, also the wrong doctor's name from the first visit, and the nursing facility was dosing incorrectly. So without examination and a complaint from the patient, we would not have found that she is receiving three blood thinners, and a doubled dose twice daily of diuretic. I sent messages to both offices to make the corrections. If that record goes with her to an ER or procedure or admission, a happy little accident could cause anything but.

I love the pup's pink legs! Happy little accidents are one of life's joys, no? But I agree, the meds, etc., not so much. I am constantly amazed how my listed meds can be mucked up at the SAME facility with a few days of the last visit....without any "intended" changes. My husband also had a mysterious diagnosis appear on his records during a period he had never had any medical care whatsoever.... And my poor mother somehow got a diagnosis of MRSA attached to her which we cannot get removed (even though the hospital acknowledged it was in error) unless she goes in and has swabs done several weeks apart. It's difficult enough getting her to her necessary appointments and out of the hospital let alone try to accomplish that....so now every time she is hospitalized, staff has to gown up before even entering her room. Gah! Fingers crossed you get things straightened out. All of us should be so fortunate to have someone like you checking over shoulders. ~Robin~

Yikes, it's scary how much was wrong on those summaries, good that you caught them. Thanks for the warning to always get those summaries, I'd never thought to do that. I like the pink legs and grey spots, the whole sampler is looking good.
